“Process Hazard Analysis”
A Process Hazard Analysis (PHA) is a systematic approach for identifying, evaluating and controlling processes and requires that PHA be conducted every five years and be documented in writing. This program describes four methods of conducting a PSH: the “What if” method, the hazard and operability (HazOp) study, the failure mode and effects analysis, and the fault tree analysis. At least one member of each PHA team must have experience in and knowledge of the specific process being analyzed.
